Observation on the therapeutic effect of TCM cauterization therapy on improving sleep quality and daytime sleepiness in patients with enlarged tonsils

Abstract: Objective To evaluate the effectiveness of TCM cauterization therapy in improving the sleep quality and daytime sleepiness of patients with grade II and Ⅲ tonsillar hypertrophy, and to provide a scientific basis for the clinical application of TCM cauterization therapy in diseases related to sleep disorders caused by significant tonsillar hypertrophy. Methods A prospective self-control design was adopted. A total of 128 patients with grade II and Ⅲ tonsillar hypertrophy were included. All patients received TCM cauterization therapy. Before treatment and 1 month after treatment, questionnaires were conducted for all patients, and the main assessment tools included: Epworth Sleepiness Scale(ESS)and Pittsburgh Sleep Quality Index(PSQI). Data were processed using R software(version 4.4.2). Results 128 patients received TCM cauterization therapy, with an average of 8.7 treatments. All patients completed the complete assessment before and after treatment. The data showed that the total score of the Pittsburgh Sleep Quality Index(PSQI)decreased significantly from(19.40±1.19)points before treatment to(5.90±1.61)points after treatment, with a statistically significant difference(P<0.001). In the seven dimensions of PSQI, statistically significant improvements were observed(all P<0.001). The Epworth Sleepiness Scale(ESS)score results showed that the sleepiness score of patients decreased significantly from(1.29±0.62)points before treatment to(0.22±0.42)points after treatment(P<0.001). Conclusion The results of this study prove that for patients with grade II and Ⅲ tonsillar hypertrophy, TCM cauterization therapy can significantly improve the overall sleep quality and effectively relieve daytime sleepiness. TCM cauterization therapy, as a minimally invasive, safe, and therapy that retains the immune function of the tonsils, has important clinical application value and promotion prospects in the treatment of sleep disorder-related diseases caused by excessive tonsillar hypertrophy, and can be an important alternative to tonsillectomy.
