Abstract:

Objective    To explore the new surgical technique to improve the treatment of laryngeal injury granuloma. Methods    Nine cases of laryngeal injury granuloma, including two males and seven females(mean 45 yr; range: 22-65 yrs old),were enrolled. All the patients underwent endotracheal intubation 1 to 3 months ago and no improvement were noted after 1-2 months of treatment, such as anti-inflammation, voice rest, or atomizing inhalation therapy. Two cases showed breathing difficulty. With microscope and suspension laryngoscope, radiofrequency ablation system  was applied to clear the granuloma from the top to the bottom without damages to the surrounding mucosa. After the surgery, atomizing inhalation of dexamethasone and gentamicin lasted for 1 week. Results    All the nine patients recovered well with only one relapse on the contralateral side, which was removed again in the second operation. Eight patients were followed up for 3 months to 2 years without recurrence. Pathological examination confirmed that the granulation tissue was squamous mucosa with chronic inflammatory response. Conclusion    Radiofrequency ablation is a safe, effective and minimally invasive method to treat laryngeal injury granuloma, without the  recurrence.
