Relationship between sinonasal anatomic variation and chronic sinusitis in endoscopic sinus surgery.

Abstract: Objective To investigate the relationship between sinonasal anatomic variation and chronic sinusitis in endoscopic sinus surgery. Methods A total of 206 patients with chronic sinusitis were selected. According to the presence or absence of chronic sinusitis, the patients were divided into observation group(n=85)and control group(n=121). The phenomenon of sinonasal anatomic variation including deviation of nasal septum, vesicular middle turbinate, inferior turbinate hypertrophy, agger nasi hypertrophy, reversed deviation of middle turbinate and uncinate process pneumatization was recorded in two groups. Results The incidences of deviation of nasal septum, inferior turbinate hypertrophy, agger nasi hypertrophy and vesicular middle turbinate in observation group were all significantly higher than those in control group(P<0.05), whereas there was no statistical significance between two groups by comparison to the incidences of reversed deviation of middle turbinate and uncinate process pneumatization(P>0.05). The upper deviation of nasal septum was very common in both groups, but the incidence of upper deviation of nasal septum in observation group was dramatically higher than in control group(P<0.05). Conclusion Deviation of nasal septum, vesicular middle turbinate, agger nasi hypertrophy and inferior turbinate hypertrophy all belong to the important structural variation in the development of chronic sinusitis, and are closely associated with the occurrence of chronic sinusitis.
