Abstract: Objective To review the timing of surgical decompression. Methods Databases including Pubmed, EMBASE, Cochrane, CNKI, Wanfang database and Weipu database were searched to collect studies on surgical timing of traumatic facial paralysis which were published from the inception of databases to March 2016. The surgical timings were classified into 4 groups: <2 weeks, 2 weeks~1 month, 1-2 months and >2 months. Results A total of 8 studies involving 145 patients(145 cases)were included. Of all patients, 56(38.6%)achieved complete recovery(House-Brackmanm, H-B Ⅰ). Good results(H-B Ⅰ and Ⅱ)were achieved in 96.8%(30/31)patients who received surgery within 2 weeks versus 64.9%(74/114)patients who received surgery after >2 weeks, showing a statistical difference(P<0.001). Conclusion The ideal timing of surgical decompression for traumatic facial paralysis is within 2 weeks after trauma. Surgical intervention performed within 2 months results in good clinical prognosis.
