摘要：

目的探讨传统鼻腔泪囊吻合术失败的原因及鼻内镜下再次行鼻腔泪囊吻合术的可行性。方法收集传统的鼻腔泪囊吻合术后失败的15例(15眼)患者行鼻内镜下再手术治疗慢性泪囊炎，并且同时治疗鼻腔的疾病(鼻中隔弯曲、鼻息肉及鼻窦炎)，并分析其失败的原因。结果 15例(15眼)患者，治愈10例(10眼)，好转5例(5眼)，原吻合术腔泪囊黏膜黏连、骨窗过小为手术失败的主要原因。结论鼻内镜下鼻腔泪囊吻合术为外切口鼻腔泪囊吻合术失败的切实可行的手术方法，术腔置丝裂霉素C可提高鼻腔泪囊吻合术成功率。

Abstract:

Objective To discuss the reason of failure of traditional darcryocystorhinostomy and the feasibility of secondary rhino-endoscopic darcryocystorhinostomy surgery. Methods Fifteen patients (15 eyes) who underwent traditional darcryocystorhinostomy which ended up in failure underwent secondary rhinoendoscopic darcryocystorhinostomy surgery for chronic dacrocystitis, simultaneously deflection of the nasal septum, nasal polyps and sinusitis were treated, and the reasons of the previous failures were analyzed. Results Out of 15 patients(15 eyes), 10 (eyes) were cured and 5 (eyes) improved, and dacrocystmucosal accretion of the cavity and too small bone windows were the reason of previous failure. Conclusions Rhino-endoscopic darcryocystorhinostomy surgery is feasible in case of failure of traditional darcryocystorhinostomy, and mitomycin administration in the operation cavity can help increase the success rate of the surgery.
