Abstract: Objective Neurilemmomas in the nasal cavity or paranasal sinuses are rare. This study aimed to improve our understanding of this phenomenon by exploring the clinical features, diagnosis, and management of neurilemmomas in the nasal cavity or paranasal sinuses. Methods Clinical records of all patients with neurilemmomas in the nasal cavity or paranasal sinuses treated at our hospital between 2012 and 2021 were retrospectively reviewed. Results Ten female and three male patients were included. One case was malignant and twelve were benign. All patients underwent surgery, and the patient with malignancy received postoperative radiotherapy. The follow-up duration of the study was 4-110 months, and the clinical follow-up data revealed no recurrences in the 12 benign cases; however, the patient with malignancy died of recurrence. Conclusion Neurilemmomas in the nasal cavity and paranasal sinuses are typically benign, with no specific clinical manifestations. In our study, the most common complaints were unilateral nasal congestion and headaches, and the most common sites were the nasal cavity and ethmoid sinus. Imaging examination is valuable in identifying the range of the lesions and in diagnosing benign and malignant tumors. Complete surgical excision using the appropriate approaches has been demonstrated to be effective, with a low recurrence rate, whereas postoperative radiotherapy is necessary for malignant cases because the risk of relapse is high.
