    Expression of gene BRMS1protein in supra-glottic laryngeal carcinoma and its clinical significance

    Objective To investigate the expression of breast cancer metastasis suppressor 1(BRMS1)protein in supra-glottic cancer and to evaluate its clinical significance. Methods The expression of BRMS1 protein was determined by using the immunohistochemical method in 60 cases of paraffin-embeded supraglottic cancer tissues and their surrounding laryngeal normal mucosa tissues(LNT). Results The expression of BRMS1 protein was positive in 35.0%(21/60) cases of carcinoma and 100%(60/60) in LNT, and it was lower in supra-glottic cancer than in normal mucosa, also it was significantly different between supra-glottic cancer tissues and the LNT(P<0.05). The expression of BRMS1 protein in supra-glottic carcinoma was related to the histological grade(P<0.05), that in a high cell differentiation was 43.6% (17/39) and in a moderate and a low cell differentiation was 19.0%(4/21), and it was related to the clinical stage that in stage Ⅱ was 72.7%(8/11)and in stage Ⅲ-Ⅳ was 26.5%(13/49), also it was related to cervical lymphatic metastasis(P<0.05), that with cervical lymphatic metastasis was 13.7% (5/38) and without cervical lymphatic metastasis was 72.7%(16/22). The 3 and 5-year survival was not significantly different between the positive and the negative groups. Conclusion Expression of BRMS1 protein in supra-glottic cancer is lower than that in normal mucosa. The decrease of BRMS1 protein expression may be related to the clinical stage, low differentiation and cervical lymph node metastasis of supra-glottic cancer.
    Laryngeal allotransplantation by stages in beagles: an initial study

    Objective To establish a model of cyno-laryngeal transplantation. Methods 10 beagles, matched-pairs in homogeneity and weight (8-10kg), were divided into 5 groups with allotransplantation. In stage Ⅰ, vascular circulation was managed with an end-to-end or end-to-lateral anastomosis of the allograft bilateral common carotid arteries with the recipient bilateral common carotid arteries, and bilateral external jugular veins. The recurrent laryngeal nerve was anastomosed end-to-end. Then, a vasculared allograft was embeded in the cervical subcutaneous of the receptor, and an immuno-depressant (cosA) was administered to the receptor based on the weight every day. In stage Ⅱ, the allograft was reimplanted with the pharyngeal and the trachea portion anastomosed when the allograft survived. Results In stage Ⅰ, the allograft survived in groups 3, 4 and 5 for up to one month and it had necrosis in groups 1 and 2 for receptor cervical infection. Conclusions The receptor beagles can tolerance the laryngeal allotransplantation by stages (laryngeal allograft fore-embed model) and the allograft can survive, however, glandular organ secretion is a troublesome problem and an immuno- long time.
    Epiglottis flaps in the reconstruction of laryngeal dysfunction following partial laryngectomy in glottic carcinoma

    Objective To investigate the effects of epiglottis flaps in re-operative reconstruction for patients with laryngeal dysfunction following partial laryngectomy. Method Nineteen patients with laryngeal dysfunction subjected to partial laryngectomies for glottic carcinoma were treated with re-operative reconstruction with epiglottis flaps. Results Eighteen patients (94.7%) were successfully decannulated 17 to 194 days after reconstruction. One patient failed because of cicatrization. Conclusion Epiglottis flaps in the reconstruction of the laryngeal cavity following partial laryngectomy are easily gained and had good efficiency.

    Objective To evaluate the effects of subtotal-patathyroidectomy and total-parathyroidectomy with parathyroid tissue transplantation in the forearm in treatment of severe secondary hyperpathyroidism. Methods The clinical data of 11 patients subjected to subtotal-patathyroidectomy (group Ⅰ) and 4 patients subjected to total-parathyroidectomy with parathyroid tissue transplantation in the forearm (group Ⅱ) were retrospectively analyzed. The PTH and Ca levels before the operations, and 1 day and 6 months after the operations were determined. Results The PTH level was remarkably decreased after operations but there was no significant difference between the post-operative 1 day and the post-operative 6 month groups. The Ca level was also significantly decreased after the operations but it only had statistic significances in group Ⅰ. Conclusion Both subtotal parathyroidectomy and total parathyroidectomy with parathyroid tissue transplantation in the forearm have therapeutic effects for renal secondary hyperparathyroidism and the effects had no significant differences in the short term.

    Objective To investigate a method for percutaneous biopsy for head and neck tumors. Methods Percutaneous biopsy was performed on 24 cases of head and neck tumors under the guidance of Simens Somatom HiQ-S CT, GE Lightspeed QX/I helical CT and with Angiomed automatic biopsy gun. Result All reached the right positions and no complications were produced. Conclusions CT-guided percutaneous biopsy of head and neck tumors has a high success rate and diagnostic accuracy rate with little complication. It is worth being clinically applied as a complementary method for endoscopy.

    Objective To explore the diagnosis and treatment of middle ear carcinoma. Method The clinical data of 19 patients with middle ear carcinoma were retrospectively analyzed. Of them, 17 patients had squamous cell carcinoma, 1 had ceruminous gland carcinoma and 1 had papilloma with malignant transformation. 14 patients received surgeries and post-operative radiotherapies, 1 radiotherapies plus chemotherapies, 2 chemotherapies alone, and 2 abandoned treatments. Result The 3- and 5-year survival rates were 31.2%(6/19)and 10.5%(2/19).Conclusion Ideal treatment is surgeries combined with radio-chemotherapies based on accurate classifications. Rational treatment has some responses following relapse.

    Objective To guide laryngeal cancer patients to rationally use antibiotics. Methods Bateria culturing was carried out in 69 isolates of laryngeal cancer following tracheotomy and the susceptibility to antibiotics was determined. Results There were much more Gram-negative bacteria than Gram-positive bacteria isolated from sputum cultures. Most of them were sensitive to Imipenem, Meropenem and Cefoperazone/sulbactam, while Staphylococcus aureus was only sensitive to Vancomycin. Conclusion Antibiotics should be reasonable used based on the drug sensitivity test. Also careful nursing following tracheotomy is important.

    Objective To explore the effects of upper airway multi-level surgeries on the swallowing-related quality of life in patients with obstructive sleep apnea hypopnea syndrome(OSAHS). Methods The SWAL-QOL questionnaire was given to 68 OSAHS patients before and 6-17 months after surgeries. Depending on whether the tongue base operations were done, the patients were divided into two groups:the multi-level operation group(n=39) and the retropalatal operation group(n=29). The swallowing-related quality of life was evaluated before and after the operations. Results Velopharyngeal insufficiency occurred in 31 patients(45.59%),choking when eating or drinking in 13 (19.1%),eating slower than normal in 14 (20.6%) and double swallows or needing great efforts in 5 (7.35%). The retropalatal operation group had significant differences before and after surgeries in eating duration and symptom frequency dimentions (P<0.01). The multi-level operation group had lower scores in berden(P=0.041), eating duration(P=0.002), symptom frequency(P=0.000), communication (P=0.007), mental health (P=0.000)and social interaction(P=0.041) after surgeries than before surgeries. Following the operations, the scores of the multi-level operation group alsowere lower than the retropalatal operation group in berden(P=0.047), eating duration(P=0.016), symptom frequency(P=0.016), communication(P=0.010), mental health(P=0.047)and social interaction(P=0.047). Conclusion Upper airway multi-level surgeries have negative effects on the swallowing-related quality of life.
    Correlation between the health-related quality of life and polysomnography results in patients with obstructive sleep apnea hypopnea syndrome

    Objectives To explore the health-related quality of life (HRQoL) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and its relationship with the polysomnogrphy (PSG) results and with excessive daytime sleepiness. Methods 64 patients with OSAHS and 50 control subjects were measured for HRQoL using the Medical Outcomes Study Short-Form 36 survey (SF-36) and Epworth Sleepiness Scale (ESS). The differences were examined by an independent-sample t test. The associations between the SF-36 and PSG results, ESS and other physical parameters were examined by simple linear correlation analysis and multiple stepwise multiple regression analysis. Results Excessive daytime sleepiness was found in OSAHS patients. 4 of 8 domains and the total scores on the SF-36 of patients with OSAHS were significantly lower than those in the controls. In OSAHS patients, 6 of 8 domains and the total scores on the SF-36 patients were significantly correlated with the ESS scores. ESS scores and the total scores on SF-36 were significantly correlated with arousal indexes on PSG. The total scores on SF-36 were significantly correlated with the SaO2<90% sleep percentages on PSG. ESS scores served as an independent factor for predicting the total scores on SF-3 which accounted for 26.9% of the total variance. Conclusions The HRQoL of patients with OSAHS decreases compared with the controls, and excessive daytime sleepiness, nocturnal durative hypoxemia, and sleep disruption seem to play important roles.

    Objective To explore the characteristics of brain auditory evoked potention (BAEP) in children with obstructive sleep apnea hypopnea syndrome with different stimulation frequencies. Method BAEP was determined in 87 OSAHS patients (the OSAHS group) and 20 controls (the control group) with different stimulation frequencies. Results(1) In children with mild and moderate OSAHS, the response threshold of wave Ⅴ and the latency values of waves Ⅰ, Ⅲand Ⅴ were normal at a stimulation of 33.1Hz; the threshold of wave Ⅴ was normal, but the latency values of waves Ⅴ and the transmission time among wavesⅠ, Ⅲ and Ⅴwas found to be prolonged in the moderate OSAHS group with a stimulation of 11.1Hz. (2) A statistically significant elevation was found when compared with the control group, the latency values of waves Ⅴ and the transmission time between waves were found to be prolonged in the severe OSAHS group with stimulations of 33.1Hz and 11.1?Hz. Conclusion Low stimulation frequency can reflect changes of brain stem functions. Pathological sleepiness in OSAHS patients may induce BAEP abnormalities, suggesting that OSAHS effect the latency values of waves and the hearing threshold.

    Objective To analyze the characteristics of polysomnography and treatment in senile patients with obstructive sleep apnea-hypopnea syndrome. Method 32-guide polysomnography was performed on 87 senile patients and 296 middle-juvenile patients with obstructive sleep apnea-hypopnea syndrome, and the results were analyzed by Polysmith. Also the 87 senile patients were treated with continuous positive airway pressure(CPAP)and an oral appliance (OA). Results The AHI was (25.97±18.23)/h in the senile patients and (46.43±27.26)/h in the middle-juvenile patients. The mean value of the NREM stage between the two groups was similar. The REM of the senile patients was higher than that of the middle-juvenile patients[(13.08±7.37)% vs (9.78±7.23)% ]. Conclusion Senile patients with obstructive sleep apnea-hypopnea syndrome should first select non-invasive treatment.

    Objective To investigate the effect of electrotome on hemorrhage following continue positive airway pressure(CPAP)and ambulatory blood pressure control in patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS) . Methods 252 patients with OSAHS and hypertension were divided into the control group and the treatment group. The control group was orally subjected to drugs to control blood pressure without CPAP and electrome, while the treatment group was subjected to infusion drugs to control blood pressure and was also treated with CPAP and electrotome in the operations. Results The bleeding was significantly less in the treatment group than in the control group(P<0.01).Conclusion Peri-operative management is important for decreasing the hemorrhage during and after operations in patients with severe OSAHS.

    Objective To determine the sites of upper airway collapse by upper airway pressure measurements in subjects demonstrated to have obstructive sleep apnea (OSA) after uvulopalatopharyngoplasty (UPPP). Methods Nocturnal polysomnography and continuous upper airway pressure measurements with ApneaGraph were performed on 10 subjects with subjective snoring or excessive daytime sleepiness after UPPP. The ApneaGraph transducer catheter contained two pressure sensors and two temperature sensors used for determination of the obstruction sites and the apnoeic events during sleep. The obstructive sites were in the upper (retropalatal oropharynx) or lower levels( the retroglossal region). The lower limit of the upper airway obstruction was determined by the pressure patterns. The constituent ratio was used to reflect the obstructive proportion of different levels. The effects of posture on apnea hypopnea index(AHI) were also analyzed. Results 3 patients had moderate OSA and 7 patients had severe OSA. 6(60%) cases had predominant sites of obstructions in the upper level(the constituent ratio of the retropalatal obstruction >50%), and 4 patients(40%) in the lower level(the constituent ratio of the retroglossal obstruction>50%). The supine AHI(66.52±22.51) was higher than the lateral AHI(47.82±21.82) and the difference between them was significant(P=0.017). Conclusion Upper airway pressure measurements can accurately identify the level of upper airway obstruction. Most patients with OSA following UPPP have the predominant obstructive sites at the retropalatal oropharynx and they tend to have more AHI in the supine position than in the lateral position.
    Epidemiology of snoring in the Qingdao area

    Objective To investigate the ratio and related factors of snoring in the Qingdao area and provide basic data for epidemiology. Method From June 2005 to December 2006, 1756 persons from 459 families were investigated by questionnaires. Results Snoring during the night was found in 59 cases (37 males and 22 females) in this group. The total ratio was 3.03%. The incidence in males was higher than in female, also it was higher in smokers than in non-smokers. Conclusion Smoke and age are risk factors for snoring.

    Objective To explore the clinical features of sudden sensorineural hearing loss(SSHL) between old and young wrinkly patients. Methods A retrospective review of 123 cases (129ears)of SSHL was carried out. All patients were classified into two groups: group A(76ears, over 60years old) and group B(53ears, younger than 60years old). All cases were treated with steroids and energy composition. Some were treated with a hyperbaric oxygen chamber and additional Batroxobin. 5 cases which failed these routine therapies were treated with intratympanic steroid therapies. A Partnership t test and Chi square test were used for analysis. Result The total effective rate of the two groups was 59.2% and 69.8% (P>0.05). The total excellence rate of the elder was significant lower than that of the young(34.2% vs 56.6%) (P<0.05). The mean duration before initiation of treatment was significantly different between the two groups(t=2.14,P<0.05). The opposite ear′s hearing threshold level was significantly higher in the elder than in the young. Diabetes and Hyperlipemia was more frequently in the group of elder than in the group of young. Conclusions SSHL in the old gradually and hidingly unobserved initiates and often causes based hearing impairment in both affected and opposite ear.Though the total effective rate is not significantly different between two groups, the excellence rate is significantly lower in the older than in the younger. Intra-tympanic steroid therapy is useful in some patients who fail routine therapy.

    Objective To improve the therapeutic effect of papilloma of external auditory canal surgery. Method Twenty-one cases were subjected to the removal of papillomas and auditory canals shaping plus pedicle skin flap or free flap transplantation under a microscope. Result Followed up for 2 years, no relapse or external auditory canal stenosis was found. Conclusion The above treatments are effective for papilloma of the external auditory canal with little relapse, infection or stenosis.
    Minimal invasive adenoidectomy under nasal endoscopy

    Objective To explore the advantages of minimal invasive adenoidectomy with an electrical cutter with suction under nasal endoscopy. Methods 50 children with chronic sinusitis, sleep respiratory disorder or hearing loss induced by adenoid hypertrophy were subjected to adenoidectomies through the nose and month under nasal endoscopy. Results The adenoid gland was completely removed and the normal anatomic character was not destroyed. Conclusions This double-route adenoidectomy under nasal endoscopy provides a direct and clear view for accurately and effectively removing the adenoid tissue with less complications.

    Objective To study the clinical manifestations, allergic characters and management of ragweed pollenosis in the Qingdao area. Methods The intra-dermal test and intranasal provocation test with ragweed pollen allergen, and specific serum IgE determination were given to patients with pollenosis. Results 85 patients were proved as ragweed pollensis in 502 patients with pollenosis based on the ARIA standard. They received step-treatment and specific immunotherapy(SIT). They were usually attacked from August to October in the pollen season, and 81.12% of them had complicated allergic conjunctivitis and 21.21% of them complicated bronchial asthma. In the 85patients, 54 were defined as intermitted rhinitis and 31 as persistent rhinitis. The response rate of SIT was 84.62%. Conclusions Ragweed pollen has become one of the major allergic pollens, so it should be eradicated to prevent pollenosis in the Qingdao area. SIT is useful in treating pollenosis and modulating the progression to asthma.

    Objective To explore the treatment method of perennial allergic rhinitis. Methods 72 patients with perennial allergic rhinitis had inferior turbinate mucosa section performed. The rank-sum test of ranked data was used to analyze the curative effect at 3 months and 1 year after the operations, the t-test of pairing data was used to analyze the stroke time of bronchial asthma 1 year before and after the operations. Results Followed up for 3 months, 66 patients had excellent effect, 5 had effect and 1 had no effect. Followed up for 1 year,28 patients had excellent effect, 35 had effect and 9 had no effect. The difference was statistically significant(u=1219, P<0.01). In 12 patients combined with bronchial asthma, the total stroke time was 71.66±63.26 h after operations and 89.25±75.65 h before operations, there was statistical significance (t=2.957, P=0.013). Conclusions Inferior turbinate mucosa section is invariably valid for perennial allergic rhinitis for a short time and the effect will be weakened with time. It is valid for patients with bronchial asthma.

    Objective To investigate the management of intractable epistaxis. Methods 101 cases suffering from epistaxis were subjected to nasal endoscopy. Coagulation using bipolar electro-coagulation or scraping the bleeding points was performed before mini-traumatic padding or modified choanal plugging. Result 78 cases were cured with a single treatment and 23 by twice using nasal endoscopy. Conclusions Hemorrhage can be preferably controlled by identifiying the bleeding points by coagulation using bipolar electro-coagulation, and those that are not identified by the bleeding points can be cured by mini-traumatic nasal padding or modified choanal plugging.

    Objective To observe the ultrastructure of surfactant lamellar bodies in the nasal mucosa. Method The nasal mucosa of 8 normal guinea pigs were observed. Result Lamellar bodies were scattered in the nasal mucosa and they were circular, lamellar- and coil-like small bodies with compact electron densities. Conclusion The nasal mucosa has lamellar bodies whose secreted surfactants play a role in nasal function and pathophysiology.

    Objectiv To explore the clinical features and treatment methods for subperiosteal orbital abscess secondary to acute sinusitis in children. Method The clinical features, computed tomographs and outcomes of different treatments for 35 children from 5 to 12 years old with subperiosteal orbital abscess secondary acute sinusitis were respectively analyzed. Results 13 cases were treated only by drugs and 22 by endoscopic drainages. The effects between the two groups were as follows: none in the former and 9 in the latter had a sight descent, 2 in the former and 14 in the latter had chemosis, 5 in the former and 17 in the latter had an eyeball displacement, none in the former and 13 in the latter had elevated intraocular pressure, and none in the former and 14 in the latter had extra-ocular movement restriction. The width of abscess in the former was significantly less than that in the latter (0.21cm vs1.35cm). Conclusions Endoscopic drainage should be applied for pediatric subperiosteal orbital abscess patients with sight descent, eyeball displacement, restriction of extraocular movements, intraocular pressure>20mmHg and abscess width>4mm.

    Objective To investigate the outcome of the wavefront-guided laser in situ keratomileusis(LASIK) in correcting low and midrange myopia. Methods The wavefront-guided laser in situ keratomileusis(W-LASIK) were performed on 48 eyes. The visual acuity, refraction and higher order aberration were determined pre-and post-operation, and the outcomes were compared with those of 42 eyes following standard-LASIK(S-LASIK). Results At 1 month after wavefront-guided surgery, compared with the BCVA of pre-operation, the UCVA was raised 2 lines in 3 eyes, 1 line in 16 eyes, was not changed in 24 eyes, and was decreased 1 line in 3 eyes. The outcomes were superior to those of the S-LASIK group. The coma following W-LASIK was lower than that following S-LASIK. In addition, the post-operative root mean square (RMSh) negatively related to the pre-operative vertical coma. Conclusions The wavefront-guided laser in situ keratomileusis can decrease the higher order aberration of myopia patients and increases the quality of vision.

    Objective To investigate and evaluate the clinical effects of cryotherapy in the treatment of Coats′ disease.Methods A total of 10 patients (11 eyes) with Coats′ disease grade 3 underwent cryotherapy. The visual acuity, retinal detachment and abnormal retinal vessels were determined. Results Fundus fluorescein angiography (FFA)showed that abnormal retinal blood vessels were destroyed and the retina re-attached in all eyes. Also the visual acuity was significantly improved. Conclusion Cryotherapy has a fairly good effect in the treatment of Coats disease.

    Objective To explore the clinical efficacy of the epithelial laser in situ keratomoleusis(EPI-LASIK) in the treatment of myopia. Methods During the EPI-LASIK for 268 cases (518 eyes), the corneal sub-epithelial valve was made by means of the nasal side for stem and LASIK was performed with Moria, then the corneal sub-epithelial valve was reset and contact lens were used for 3 to 5 days. All the patients were followed up for 3 to 6 months, and the postoperative symptoms, the healing conditions of cornea, naked vision, diopter EPI and Haze were observed. Results During the operations, the corneal sub-epithelial valve was made well, and 5 days after operations, the epithelium of 486 eyes healed well and contact lens were not used. 30 eyes had corneal edema in fragmentis and they had no adverse effects when the lens were picked off on time. 2 eyes had corneal edema of large areas and healed well when the lens were picked off 2 days later. The best corrected visual acuity did not decrease 3 months after operations, and it was raised 1 or 2 lines in 58 eyes.The cornea Haze of 54 eyes was 0.5, of 16 eyes was 1 and the rest were transparent 3 months after operations. 384 eyes were followed up for 6 months, 22 had Haze 0.5 and 8 had Haze 1. Conclusion EPI-LASIK has the benefits of safety, stability, and good predictability.

Method The refractive diopter, corneal curvature, corneal depth and intraocular pressure were determined and analyzed. Result There were some correlations among the above factors in myopic eyes. Conclusion There are some compensatory mechanisms in myopia.

    Objective To explore the effects of the blink reflex(BR) and electrical neurography(ENG) in the evaluation of Bell′s facial palsy. Method Haishen nerve examination instruments were used to stimulate the super-orbital nerve to record wave R1and R2 of the ocular orbicular muscle in the same sides and wave R2 of the ocular orbicular muscle in the opposite sides. A concentric needle electrode was used to stimulate the facial nerve to record the latent phase of movement and the scope of wave M. Also the BR and ENG were determined and compared with the controls. Result BR was abnormal in 35 cases of Bell′s facialpalsy in different degrees, and ENG was abnormal in some of the cases. Conclusions Both BR and ENG can reflex the degree, location and prognosis of the facial nerves.BR is more important in early diagnosis and prognosis than ENG,but their combination is more valuable than when only one is used.
