Objective: To construct recombinant B71 retrovirus expressing vector by clone of B71 gene from human blood and construct the recombinant plasmid of B71. Method: Primers for B71 were designed and synthesized according to the sequence of human B71 gene derived from GenBank. The full length cDNA of B71 was cloned by RTPCR techniques. The recombinant plasmid pGEMT B7 were constructed by DNA recombinant techniques. Results: The length of RTPCR product coincided with that of authors′ anticipation(889 bp), and the recombinant plasmid was confirmed by restriction enzyme digesting with EcoRI and Hind III. The sequencing result of the cDNA was identical to the sequence of B71 cDNA in GenBank, and the full length cDNA of human B71 was successfully inserted into the pGEMT vector. Conclusion: The successful cloning of human B71 cDNA, as well as the construction of its retrovirus expressing vector enables us to further investigate the role of B71 in tumor immunogene therapy.

[ABSTRACT]Objective: To construct an expressing vector (pSIRENShuttleEGFR ) of RNAi in order to suppress gene expression of epidermal growth factor receptor (EGFR). Methods: Three 69 basepair oligos coding hairpin RNA which specifically aimed at EGFR gene were chemically synthesized and annealed, then inserted into the downstream of U6 promoter of vector to construct RNAi plasmid (pSIRENShuttleEGFRs). Sequence analysis was used to identify the correction of recombinant vector. RT-PCR was performed on the CNE cell line transfected by pSIRENShuttleEGFRs. Results: Sequencing analysis demonstrated that 69 bp had been inserted into expected site and the insertion sequence was exactly correct in recombinant pSIRENShuttleEGFRs vectors. It was showed that usingthe plasmid vector pSIRENEGFRs to induce RNAi suppressed the expression of endogenous EGFR in CNE human nasopharyngeal epidermal carcinoma cells. As a consequence, the mRNA of EGFR was decreased. Conclusion: RNAimediated inhibition of EGFR gene induced by shRNA expressing plasmid vector may constitute a useful approach in the treatment of human nasopharyngeal epidermal cancer.

[ABSTRACT]Objective: To observe normative auditory brainstem response (ABR) latencyintensity functions and the threshold and provide reference for combined clinical use of air and boneconducted ABR. Methods: ABR absolute thresholds and latencies(Ⅴwaves)were obtained from 56 ears of 28 young persons with normal hearing using both the EARTONE insert earphone and the Radioear B71 bone viboratory. Results: The air and boneconducted waveⅤlatencyintensity functions exhibited increased latencies as intensity decreased. The boneconducted clicks yielded somewhat longer latencies than the airconducted ones. Conclusions: (1) The waves of air and boneconducted ABR are similar, and more information on ascertaining the presence and magnitude of a conductive hearing loss can be obtained when both air and boneconducted ABR wave V latencyintensity functions are administered. (2) When the threshold of boneconducted hearing exceeds approximately to 40 dBHL, no BCABR responses can be observed. (3) Masking is often necessary when administering boneconducted ABR test, but intensity of masking should not exceed to over 60 dBSPL.

Objective: To explore the application of MRI in diagnosing sudden deafness. Methods: No pathological symptoms were found on 21 patients with sudden deafness by computerized tomography and they accepted systematical treatment for one week but did not get any improvement. Then they were examined by magnetic resonance imaging. Their medical data, including hearing level and treatment course were retrospectively analyzed. In addition, the related literatures were reviewed. Result: Out of the 21 patients, 5 showed pathological changes by magnetic resonance imaging. Conclusion: To those sudden deafness patients whose symptoms do not alleviate after systematical treatment of one week, magnetic resonance imaging should beapplied to discover some covered pathological changes.

Objective: To investigate the surgical method and the curative effect of inferior turbinate function preservation for chronic hypertrophic rhinitis. Methods: Rhinoplasty of inferior turbinate by endoscopic sinus surgery was operated on 60 patients, on 20 of whom cuneal tissues were performed under mucous membrane of inferior turbinate and partial bone resection, and on 40 of whom cuneal tissues resection was performed. The time of debridement from mucous membrane and the pH value of secretion in nasal cavity were determined before and after operation. Results: Little pain and bleeding were found after operation. The time of debridement for mucous membrane and the pH value of secretion in nasal cavity turned to normal in two months. Conclusion: Rhinoplasty of inferior turbinate by endoscopic sinus surgery is a new operation skill with good effect for chronic hypertrophic rhinitis.

Objective: To evaluate the effect of tympanic membrane indwelling catheter under microscope on secretory otitis media after radiotherapy for nasopharyngeal carcinoma. Method: Twentyseven cases(18 males and 9 females) were treated and followed up. Result: Nine(23.1%) out of 39 ears were cured, and the total efficiency rate was 87.2%(34/39). Conclusion: Tympanic membrane indwelling catheter under microspoce is a favorable way to straighten the eustachian tube function with weak damage.

Objective: To explore the possible mechanism of glucocorticoids on nasal polyps. Methods: Eotaxin protein expression was measured by immunohistochemical techniques in 5 nasal polypous tissue specimens following 30 mg predisone oral doses per day for 7 days, 5 nasal polypous tissue specimens following 10 mg dexamethasone intravenous drops, 5 without any treatment and compared with the results from the inferior turbinate tissue specimens. Results: The protein level of eotaxin significantly elevated in the nasal polyps compared to the controls (P<0.01), whereas it significantly decreased in the treatment group compared with the untreatment group(P<0.01), and similar to the controls(P>0.05). Conclusion: Because eotaxin actively participates in the process of eosinophilic inflammation, glucocorticoids play an important role in antiinflammation by downregulation of eotaxin.

Objective: To observe the longterm curative effect of endoscopic sinus surgery(ESS). Method: Seventyfive cases(125 sides) of chronic sinusitis and nasal polyps were treated with ESS, and were followed up for 1 year after operation. Results: Fourtyeight cases were cured (64.0%), 20 achieved symptomatic relief(26.7%), 7 remained failure(9.3%), and the response rate was 90.7%. The major operative complication was adhesion of nasal cavity. Conclusion: ESS has an ideal effect on chronic sinusitis and nasal polyps. Careful treatment of middle turbinate, inferior turbinate and nasal septum, and regular followup are important to reduce complications.

Objective: To study the effect of erythromycin on apoptosis in nasal polyps. Methods: Epithelial cells from 30 nasal polyps and 6 inferior turbinates were cultured in Dulbecco eagle and Ham F12(1:1), which were divided into two groups, one with erythromycin, and the other without. Apoptosis was detected by terminal deoxynucleotidyl transferasemediated dUTP nickendlabeling. Results: The apoptosis index (AI) of epithelial cells in nasal polyps after cultured for 1,3,5 days was (33.23±6.50)%,(38.21±7.22)% and (52.63±7.86)% respectively. The AI of epithelial cells in inferior turbinate was (31.02±5.60)%,(32.13±7.15)% and (39.64±7.48)% respectively. There was a significant difference between the two groups at day 5 after culture(P<0.05). Conclusion: Erythromycin promotes apoptosis of epithelial cells from nasal polyps.

Objective: To assess the surgical method in treating nasopharyngeal fibroangioma encroaching on fossa pterygopalatina. Methods: By the approach from palatum durum chilogum, the maxillary tuberosity and part of retrolateral wall of the genyantrum were resected, and the external and internal plates of the processus pterygoideus were exposed, then the processus pterygoideus and the whole tumor tissue were resected. Result: No recurrence was found during the followup of 2~5 years. Conclusion: The approach from palatum durum chilogum seems to have a salutary effect on resecting the maxillary tuberosity and part of retrolateral wall of the genyantrum.

Objective: To evaluate the efficacy and safety of mizollen for seasonal allergic rhinitis. Methods: One hundred outpatients were randomly indivied into mizollen and loratadine groups following 10?mg/d mizollen or loratadine for 14 days. The clinical efficacy was evaluated by symptom score reducing index (SSRI). Results: Mizollen was superior to loratadine for nasal obstruction in seasonal allergic rhinitis (SAR). No reverse incidences happened. Conclusion: Mizollen is effective and safe for seasonal allergic rhinitis.

Objective: To compare the curative effect of three approaches for inverted papilloma of the nasal cavity and paranasal sinuses. Methods: Lateral rhinotomy was performed in 9 cases, extended sublabial incision in 5 cases, and endoscopic sinus surgery in 21 cases. Result: After a followup of 6 to 36 months, relapse was found in 2 patients with lateral rhinotomy, 1 with extended sublabial incision and 2 with endoscopic sinus surgery. Conclusion: Compared with the other two approaches, endoscopic sinus surgery has more advantages, such as clear field, correct operation scope, less bleeding and good effect.

Objective: To explore the curative effect of longterm lowdose clarithromycin for chronic sinusitis. Methods: Longterm lowdose clarithromycin was administered in 63 patients with chronic sinusitis from April 2002 to January 2003. The symptoms, nasal endoscopic examination,CT scan of nasal sinuses and the pathology of sinus mucosa were assessed 3 and 6 months later, and the saccharine transit time (ST) was measured to assess the transporting function of nasal mucosa before and after treatment. Results: The 3 and 6month response rates of stuffy nose, runny nose, headache, sense of smell, nasal endoscopy, CT scan and pathology were 66.7% and 73.0 %, 65.1% and 74.6%, 73.5% and 85.3%, 30.0% and 35.0%, 55.8% and 69.2%, 61.4% and 68.4%, 38.5% and 61.5% respectively. The mean ST was 35.5±6.3 minutes before treatment, and 27.8±6.6 minutes and 19.7±7.1 minutes at the 3rd and 6th months. Conclusion: Lowdose clarithromycin for a long term has a salutary effect on chronic sinusitis patients.

Objective: To evaluate the effect of combined modality therapy for children with chronic sinusitis. Method: Combined modality therapy including drug, drainage and surgery was performed on 140 children with chronic sinusitis. Results: During a followup from 6 to 12 months, 77 patients were cured, and 33 were on the mend. Conclusion: Combined modality therapy achieves good effect for chronic sinusitis in children.

Objective: To investigate the common nasal bleeding points and the management for intractable epistaxis. Methods: The bleeding points, surgical techniques and the curative effect were studied retrospectively in 68 patients, in whom the hemorrhage was not controlled with standard nasal packing. Results: The bleeding points were found in the following different sites: superior wall of inferior nasal meatus, 27.9% (19/68); olfactory cleft of nasal septum, 20.7% (14/68); posterosuperior wall of middle nasal meatus, 16.1% (11/68); sphenopalatine artery zone of posterior middle turbinate, 10.3%(7/68); posterior of nasal septum, 10.3%(7/68); superior wall of nasopharynx, 5.9%(4/68); Woodruff venous plexus, 4.4% (3/68) and the uncertain, 4.4% (3/68). Epistaxis was well controlled in 65 cases by electrocoagulation, and in 3 cases by gelfoam packing. There were no complications during a followup for 1 3 months. Among the 68 patients, bleeding was stopped in 59 ( 86.7%) after once treatment, while in 9 cases(13.3%) after twice. Conclusion: Endoscopy provides an effective and safe way to visualize and manage the deeplysited bleeding points.

Objective: To explore the role of nasal endoscopy in the diagnosis of rhinosinusitis or adenoiditis in children. Methods: A total of 112 children with possible rhinosinusitis was recruited between December 2003 and Octorber 2004. Besides routine examination, rigid nasal endoscopy was performed in all the children. The secretion in the middle meatus was collected and cultured in patients having infection. Results: Endoscopy was successful in 95 children, 20 of whom was diagnosed as rhinosinusitis, 7 as adenoiditis, and 26 as both. Children with rhinosinusitis or adenoiditis ranged from 2 to 5 years old. About 86.8% of the patients was found bacteria in their secretion. Conclusion: Nasal endoscopy has advantages of less trauma, easy performation, and efficacy on rhinitis and sinusitis, and it is proposed to be a routine examination for children with suspicious diseases in nasal cavity and nasopharynx.

Objective: To evaluate the role of glucocorticoids locally applied in chronic sinusitis and nasal polyps after endoscopic surgery. Methods: A total of 300 patients with chronic sinusitis and nasal polyps were randomly divided into treatment and control groups, in which local glucocorticoid hormone plus routine treatment and routine treatmeat alone were given respectively after operation. Results: During a followup from 6 to 12 months, 152(90.5%) patients were cured, 12(7.1%) were relieved and 4(2.4%) were suffering from nasal polyps again in the treatment group, whereas 107 (81.1%), 12(9.1%) and 13(9.9%) respectively in the control, the difference between the two groups was significant (P<0.05). Conclusion: Glucocorticoids locally applied in sinusitis and nasal polyps after endoscopic surgery are effective for preventing the recurrence.

Objective: To evaluate the effect of operation for improving ventilation of nose in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods: The Epworth sleepiness scale(ESS) and polysomnography(PSG) were assessed pre and postoperatively in 71 OSAHS patients with nasal anatomical stricture by using a flexible fiberoptic endoscope and a video recorder equipment, and septal surgery with or without inferior turbinectomy was performed. Results: ESS decreased from 14.8±5.2 to 9.7±4.6 after surgery (t=6.19, P=0.00). According to the results of postoperative polysomnography, 8 cases in evident efficiency, 29 efficiency and 34 nonefficiency. Improvements in sleep architecture were observed after nasal surgery, and there was a significant reduction in awakening and nonrapid eye movement (NREM) sleep 1 phase, but a significantly increase in NREM sleep 1 phase and rapid eye movement (REM)sleep phase(P<0.01). There were no significant changes in NREM 3+4 phase(P>0.05). Nasal surgery resulted in a significant decrease in apnea index (AI), hypopnea index (HI), microarousal index and snoring index, but a significant increase in lowest oxygen saturation levels (P<0.01). The number of apnea and hypopnea episodes per hour and the body mass index (BMI) did not change significantly (P>0.05). Conclusion: Nasal surgery has a limited efficacy in adult patients with OSAHS. Correction of the obstructed nasal airway should certainly be included in the overall treatment for OSAHS. The secondary surgery (such as uvulopalatoplasty) may be considered after a thorough evaluation by postoperative PSG .

Objective: To retrospectively analyze the results of diagnosis and treatment and enhance the diagnostic and therapeutic level of nasoorbit related diseases. Method: Thirtyfive cases of nasoorbital diseases were analyzed retrospectively. Results: Of the 35 cases, rhinogeous orbital phlegmon was found in 6, traumatic nasoorbital and optic canal fracture in 4, mucocele of nasal sinus in 11, inverting papilloma of nasal cavity and nasal sinus in 3, and malignant tumor in 11. The main pathologic characteristics were as follows: bony absorption of orbit wall in inflammation disorder; compression, displacement and absorption of orbit wall in benign tumor; and bony destruction in malignant neoplasm. Conclusions: Nasal and orbital organs are closely anatomically related. CT and MRI are important in early diagnosis and treatment of nasoorbital related diseases. Treatment for benign tumors is mainly by operation, while malignant neoplasms should be treated by combination of operation and radiotherapy, etc.

Objective: To explore the value of volume rendering(VR) threedimensional reconstruction in diagnosis of temporal bone destruction by high resolution CT(HRCT). Methods: Fifteen cases of chronic otitis media with cholesteatoma were scanned with HRCT, whose original data were processed with VR and bone destructions observed in the images generated by this technique. Results: In all cases, 3DCT clearly delineated the destructions of the bone caused by cholesteatoma. The 3DCT generated images provided spatial relationships, which were not easily appreciated on two dimensional CT. Intraoperative bony destruction findings correlated with 3DCT findings. Conclusion: VR threedimensional reconstruction by HRCT has an important value to evaluate the invasiveness of cholesteatoma to temporal bone.

Objective: To explore the best surgical approach for parapharyngeal space tumors. Methods: The clinical data of 46 patients with parapharyngeal space tumors were restrospectively analyzed. Tumors were resected through intraoral and pharynx approach in 11 patients, transcervical approach in 23, transparotid approach in 2, transcervicaltransmandibular approach in 7, and infratemporal approach in 3. Results: Tumor was completely and partially resected in 43 and 3 patients, respectively. Facial nerves were injured inducing inadequate peripheral paralysis during operation in 2 patients, Horner′s syndrome was found in 2 after operation, and massive haemorrhage happened in 1 during operation. During a followup of 3 to 5 years, 2 patients died, and others were still living and in good health. Conclusion: Different surgical approaches should be adopted according to the site of the tumor so as to resect the parapharyngeal space tumors totally and safely.

Objective: To analyze the impact of chemotherapy on esthesioneuroblastoma and illustrate the suitable chemotherapy modality. Methods: A retrospective chart review of patients with esthesioneuroblastoma treated with chemotherapy at our department from Mar. 1993 to Jun. 2004, including age, gender, duration, Kadish staging, Hymas grade,therapy modality and outcome, was analyzed. Results: Four male cases, averagely aged 31.5 years, were treated. All patients were in Kadish C stage, and 1 in Hymas grade 2, 2 in Hymas grade 3, and 1 in Hymas grade 4. The main components of chemical drug included etoposide and platin. Chemotherapy was used as primary therapy on 1 case with grade 2, and the patient was still alive with tumor at the end of followup of 6 months. Chemotherapy was used on 3 cases as salvage therapy. Among them, 2 cases with grade 3 were alive without tumor after followups of 48 months and 6 months respectively, and 1 case with grade 4 was alive with tumor after followup of 12 months. Conclusions: Adoption of chemotherapy to treat esthesioneuroblastoma is effective, and the Hymas grade of esthesioneuroblastoma affects the response to chemotherapy. The components of chemical drug should include etoposide and platin.

Objective: To investgate the different activity of human epidermal growth factor (hEGF) and transforming growth factor β2 (TGFβ2) on wound healing in rabbit corneal endothelial cells and the expression of p27Kip1 and Cdk4. Methods: Wound was created in confluent secondary cultured rabbit corneal endothelial cells which was added in different doses of hEGF and/or but 10 ng/ml TGFβ2, no hEGF or TGFβ2 was added in the controls. The wound area was evaluated by computer on 1, 3, 5 and 7 days after wounding, moreover the subcellular localization of cyclin dependent kinase 4 (Cdk4 ) and p27Kip1(p27) were determined by immunofluorescent staining. Results: hEGF enhanced the wound healing and Cdk4 in a dosedependent manner and the responses peaked at 10~40 ng/ml. TGFβ2 inhibited the wound healing and upstreamed p27 in the cultured rabbit corneal endothelial cells. hEGF could be used to cure the wound following 24 hours usage, but TGFβ2 could not; whereas at the same time or 24 hours after adding TGFβ2, hEGF had no effect on the wound. Conclusion: hEGF of certain concentration enhances the wound healing in cultured rabbit cornea endothelial cells and upstreams the expression of Cdk4. Exogenous TGFβ2 inhibits the wound healing in cultured rabbit cornea endothelial cells and upstreams the expression of p27.
