Abstract: Objective To investigate the clinical features and the selection of diagnosis and treatment methods for Madelung's disease. Methods Clinical data of a patient with Madelung's disease with scrotal fat accumulation treated surgically in the author's hospital and the related literature were analyzed. Results The patient underwent surgical resection of the adipose tissue and was followed up for half a year. There was no recurrence during the follow-up period. Conclusion Madelung's disease mainly occurs in middle-aged men with a long-term drinking history. It is characterized by the presence of multiple, progressive, excessive lipomas with a symmetrical distribution. During physical examination, the accumulation of abnormal fat mass in other parts may be observed. Timely referral to the corresponding department for intervention is important. To avoid recurrence, surgical resection is currently the preferred treatment. Surgery also aims to improve the patient's appearance and any functional impairment. Complications such as occult penile syndrome, sexual dysfunction, or testicular atrophy should be avoided in patients with scrotal fat accumulation.
