孤立先天性视网膜动静脉畸形合并玻璃体出血和内界膜下出血1例并文献复习

摘要/Abstract

摘要： 目的报道1例罕见的孤立先天性视网膜动静脉畸形合并玻璃体出血和内界膜下出血。方法 1例19岁患者因左眼突发无痛性视力下降2 d行全面眼科检查,结合病史,综合多模态影像结果,临床诊断为孤立先天性视网膜动静脉畸形和Valsalva视网膜病变,给予观察随访。结果随访3年后,患者左眼病情基本稳定,最佳矫正视力提高至1.0。结论先天性视网膜动静脉畸形虽然较为稳定,但可能也是Valsalva视网膜病变的危险因素之一,需长期随访,一旦发现并发症,应及时治疗,避免严重后果。多模态影像可提供更全面的信息,有助于视网膜动静脉畸形的诊断、治疗和随访。

Abstract: Objective To describe an uncommon case of vitreous hemorrhage and sub-inner limiting membrane hemorrhage(sub-ILM)in isolated congenital retinal arteriovenous malformation. Methods A 19-year-old patient with a 2-day history of sudden painless vision loss in her left eye and underwent comprehensive ophthalmic examinations. Based on medical history and multimodal imaging, she was clinically diagnosed to have isolated congenital retinal arteriovenous malformation and Valsalva retinopathy. She was then observed and advised for follow up. Results After 3 years of follow-up, the patient's left eye was stable, with an improved best corrected visual acuity of 1.0. Conclusion Congenital retinal arteriovenous malformation is a relatively stable condition that requires long-term follow-up. Moreover, it may also be one of the risk factors of Valsalva retinopathy. Once complications are found, appropriate treatments should be provided to avoid serious consequences. Multimodal imaging provides clinicians with more comprehensive information, which is helpful in the diagnosis, treatment, and follow-up of retinal arteriovenous malformations.
