一种新型青光眼植入物——聚砜引流管的生物相容性研究

摘要:

目的      研究自行设计的新型青光眼房水引流物在兔眼上的生物相容性。方法      新西兰大白兔6只,每只兔随机取一眼作为实验眼,另一眼作为对照眼。实验眼行巩膜瓣下植入聚砜引流管,一端植入前房0.5cm;对照组眼单纯做巩膜瓣。术前和术后的1、2、 4周,测量眼压并观察眼前节形态,术后1周进行超声生物显微镜检查,术后第1、2、4周分别取2只兔,摘除双侧实验眼与对照眼各2只,行病理HE染色检查,观察兔眼局部组织对植入物的反应。结果      术后第1天,实验眼和对照眼角膜透明, 50%试验眼前房出现Tydall征(+),12.5%对照眼前房出现Tydall征(+),所有眼均未见纤维素性渗出。术后1周,实验眼与对照眼Tydall征均为(-)。实验眼与对照眼术后早期的病理组织学改变相似,都表现为炎症细胞浸润,成纤维细胞逐渐增多,未见异物巨细胞。结论      聚砜引流管具有良好的生物相容性,植入兔眼后未见局部组织排斥反应。

Abstract:

Objective      To observe the biocompatibility of a new glaucoma drainage implant (GDI) made of polysulfone in rabbit eyes. Methods     Six white New Zealand rabbits were operated on. The right or left eyes  randomly served as the experimental group and the control group. In the  experimental group, the proximal end of the GDI was inserted into the anterior chamber while the distal end was placed under the scleral flap. The control  group was performed with a scleral flap. Slit-lamp follow-up and intraocular pressure measurements were recorded before the operation and 1, 2, 4 weeks after the surgery. Bilateral eyes were observed with ultrasound biomicroscopy(UBM)  at 1 week after surgery. At 1, 2, 4 weeks after surgery, two rabbits were killed and their eyes were enucleated and histologically processed, leaving the device in situ when sectioning.  Local tissue reactions after the GDI implanted into the rabbit eyes were observed. Results     In the first day after surgery, 4 eyes (50%) showed Tyndall sign in the experimental group and 2 eyes (12.5%) showed Tyndall sign in the control group. No eye had cellulose exudation. One week postoperatively, the Tyndall sign disappeared. Histological results showed no significant difference between the experimental group and the control group.  Inflammatory cell infiltration and collagenoblast increase happened in the two groups, but no foreign-body giant cells appeared. Expression was not significantly different between the experimental and control groups. Conclusions     GDI made of polysulfone has good biocompatibility. It has no acute rejection in local tissues.
