鼻内镜下经咽鼓管鼓室内置管注药治疗梅尼埃病

Intratympanic Gentamicin through an inserted catheter for Meniere′s disease

摘要:

目的   观察鼓室内置管注入庆大霉素治疗梅尼埃病的疗效。方法   在鼻内镜引导下经咽鼓管向鼓室内置入硬膜外麻醉导管,留置1周左右,期间注入庆大霉素注射液,并同期口服类固醇激素以保护听力。观察用药后1、3、7d及3个月后的疗效。结果   庆大霉素注射后1d内即可缓解患者眩晕症状,1周内症状完全缓解,3个月内复发率极低。结论   既往大多数实验集中在庆大霉素对内耳细胞损伤作用的研究,而忽视了其治疗作用,我们认为内耳灌注氨基糖苷类抗生素可以有效治疗梅尼埃病。

Abstract:

Objective   To treat meniere′s disease by intratympanic gentamicin through an inserted catheter and to explore its effect. Methods   A catheter was put into the tympanum through an eustachina tube under nasal endoscopic visualization and it was kept for almost one week. Gentamicin was injected after insertion of catheters into the tympanum and oral steroid hormone was used to protect audition. Patients were observed on the first day, the third day, the seventh day and in three months. Results   After injection of gentamicin, vertigo was relieved the next day, and completely relieved within a week while the recurrence rate was extremely low. Conclusion   Most of retrospective experiments have focused on research about the damage effect of gentamicin to the inner ear cells without paying attention to its therapeutic effect. It was found that it is effective to cure meniere′s disease by intratympanic aminoglycosides antibiotics.
