绝经后梅尼埃病女性雌激素水平与听-前庭功能关系的研究

Correlation between levels of estrogen and auditory and vestibular functions in post-menopause with Meniere′s disease

摘要:

目的   探讨绝经后梅尼埃病(MD)女性患者雌激素水平变化与高刺激率ABR、听-前庭功能之间的相关性。方法   回顾分析2010年9月~2011年10月在山东省立医院眩晕专科诊治的绝经后MD女性患者43例(研究组)的临床资料,30例绝经后健康查体妇女作为对照组,比较并分析两组血清雌二醇(E2)水平,高刺激率听性脑干反应检查(ABR)、纯音测听、冷热试验的结果,对研究组血清E2水平与高刺激率ABR、纯音测听、冷热试验进行相关性分析。结果   研究组雌激素水平明显低于对照组(t=2.60,P<0.05),组间比较差异有统计学意义;高刺激率ABR研究组中Ⅰ~Ⅴ波潜伏期明显高于对照组(左耳P<0.05、右耳P<0.05,差异有统计学意义)。冷热试验患侧耳单侧功能下降值研究组明显高于对照组(P<0.01),差异有统计学意义。纯音测听语音频率(500Hz、1KHz、2KHz、3KHz)平均听阈研究组较对照组明显升高(左耳P<0.01、右耳P<0.01),组间差异有统计学意义。研究组中E2水平与高刺激率ABR呈负相关(左r=-0.229,P<0.05;右r=-0.220,P<0.05),与双温试验UW值呈负相关(r=-0.328,P<0.05),与平均听阈无相关性。结论   雌激素水平下降可致内耳微循环障碍,同时雌激素水平与前庭功能呈正相关关系,雌激素在某种程度上影响着绝经后女性MD的发生、发展。

Abstract:

Objective   To investigate  the correlation between the levels of estrogen and high stimulus  rate  -auditory  -brainstem   response  test, and auditory and vestibular functions in post-menopause  with Meniere′s disease. Methods   43 patients with Meniere′s disease served as the experimental group and  another 30 post-menopausal healthy women as the control group, both had the levels of estrogen and pure tone audiometry determined, and a high  stimulus  rate   auditory   brainstem  response  test (ABR) and  a bithermal  caloric  test were performed. Results   The mean level of E2 was significantly lower in the experimental group than in the control group(t=2.60, P<0.05). The mean level of high  stimulus  rate   ABR  was significantly higher in the experimental group than in the control group(left ear: P<0.05; right ear: P<0.05). The mean level of unilateral weakness was significantly higher in the experimental group than in the control group(P<0.01). The mean level of average hearing threshold was significantly higher in the experimental group than in the control group(left ear P<0.01; right ear P<0.01). The E2 level in the experimental group was negatively correlated with its high  stimulus  rate  ABR level(left r=-0.229, P<0.05; right r=-0.220, P<0.05), and the E2 level in the experimental group was negatively correlated with  its unilateral weakness level(r=-0.328, P<0.05), but the E2 level in the experimental group had no correlation with its average hearing threshold.  ConclusionT   he present work suggested that the decrease of estrogen level might be lead to microcirculatory ischemia of the inner ear, and the estrogen level was positively correlated with vestibular function. Estrogen affected  occurrence and development of Meniere′s disease in post-menopause to some extent.
