摘要: 目的 比较病原菌培养和涂片镜检诊断慢性化脓性中耳炎合并外耳道真菌感染的准确性,评估曲安奈德益康唑乳膏的治疗效果、其应用于鼓膜穿孔患者的安全性以及真菌感染控制后鼓膜修补手术效果。 方法 将44例慢性化脓性中耳炎鼓膜穿孔合并外耳道真菌感染患者列为研究对象,进行病原菌培养和涂片镜检,比较两种方法的阳性率,并行纯音测听检测。使用曲安奈德益康唑乳膏涂抹治疗后再次行涂片镜检和纯音测听检测,评估治疗效果。根据是否感染真菌分为真菌组与对照组,对照组为35例慢性化脓性中耳炎行耳内镜下鼓膜修补患者,真菌组镜检阴性后32例行耳内镜下鼓膜修补术,比较两组的鼓膜愈合率、愈合时间、干耳时间、听力情况。 结果 病原菌培养检测出真菌感染的阳性率为73.7%,其中常见的真菌种类包括曲霉菌和丝状真菌,合并细菌感染(26.3%);涂片镜检的阳性率为77.5%,两种检查方法的阳性率差异无统计学意义(P=0.695)。使用曲安奈德益康唑乳膏涂抹治疗后,经耳内镜检查和涂片镜检,患者真菌均完全控制,涂片镜检均为阴性,治疗前后纯音测听结果差异具有统计学意义(P<0.001)。真菌感染控制后行鼓膜修补术,与单纯穿孔组比较,两组鼓膜愈合率(P>0.999)、愈合时间(P=0.593)、干耳时间均无统计学意义(P=0.945)。 结论 涂片镜检和病原菌培养都具有较高的阳性率。曲安奈德益康唑乳膏治疗鼓膜穿孔合并外耳道真菌感染的患者疗效确切,无明显耳毒性。真菌感染控制后,患者鼓膜修补手术的成功率无明显改变。
