Abstract: Objective Peripapillary hyperreflective ovoid mass-like structures(PHOMS)are an important imaging biomarker that has emerged with the advancement of multimodal imaging technologies. This review, for the first time, adopts axoplasmic stasis as a unifying pathological framework to systematically elucidate the multimodal imaging features and evolutionary rules of PHOMS, clarify their extensive association with various optic nerve and systemic diseases, and highlight the high detection rate of PHOMS in children and adolescents as well as its correlation with optic disc morphology. Meanwhile, this paper systematically establishes a differential diagnostic framework for distinguishing PHOMS from optic disc drusen and defines the potential application value of PHOMS in monitoring disease activity and evaluating prognosis. Notably, this review summarizes the current controversies and unresolved issues in the field and outlines future research frontiers, such as artificial intelligence-assisted quantitative analysis, aiming to provide a more systematic theoretical framework and innovative perspectives for the clinical interpretation of PHOMS and related research.
