上气道变应炎症对下气道的影响

Effects of upper airway allergic inflammation on the lower airway

摘要: 目的通过豚鼠实验模型研究上气道变应性炎症对下气道的影响。方法12只豚鼠分为2组,每组6只,鼻炎组豚鼠给予鼻腔卵蛋白/氢氧化铝滴注(连续10?d,每天2次)。间隔1周后,鼻腔给予2%卵蛋白每日滴注,连续7?d;对照组均使用生理盐水替代卵蛋白。最后一次激发后10?min,对喷嚏、 抓鼻、 流涕等症状量化评估,观察肺部症状;抽取动物血清,ELISA方法测定IgE和IL5水平;计量鼻腔黏膜及肺部黏膜嗜酸粒细胞浸润数量,观察鼻腔和肺组织的病理改变。结果鼻炎组豚鼠明显出现喷嚏、鼻痒、挠鼻、流涕等典型症状,但肺部哮喘症状不明显;血清IgE、 IL5水平升高;鼻腔和肺部黏膜见多量嗜酸粒细胞浸润,且出现病理结构变化。对照组未出现上述变化。结论上气道变应性炎症可导致下气道发生类似的炎症反应,并导致免疫学改变。

Abstract: To observe pathological and immunological changes of the lower airway in intranasal ovabumin induced animal models of allergic rhinitis in guinea pigs. Methods12 Hartley guinea pigs (250300?g) were randomly divided into two groups (the control group and the allergen exposure group). Guinea pigs were intranasally sensitized with ovalbumin absorbed on aluminum hydroxide twice a day for 10 days, and after one week they were challenged with 2% ovalbumin solution once every day for a total of 7 times. The negative control group was shamsensitized and shamchallenged by PBS. The symptoms of allergic rhinitis as well as asthma were evaluated. Eosinophils infiltration and pathological changes of the nasal and lung tissues were observed. Levels of IgE and IL5 of the serum were determined by the Elisa method. ResultsIn the allergen exposure group, symptoms of sneezing, nasal scratching, nasal and rhinorrhea were induced after ovalbumin challenge. IgE and IL5 levels increased after challenges. Eosinophil infiltration in the mucosa of the nose and the lungs was more obvious in the allergen exposure group in comparison with the negative control group (P<0.001). Pathological changes were also founded in the nose and lung tissues of the allergen exposure group. ConclusionIt is clear that antigenic stimulation of the upper part of the respiratory mucosa has ripple effects on the lower airway and leads to changes in immunology, which supports the united airway concept.
