Research progress in mechanism and treatment of obstructive sleep apnea in heart failure

Abstract: Obstructive sleep apnea(OSA), an independent risk factor for heart failure(HF), plays a significant role in ventricular remodeling and HF prognosis through multiple pathophysiological mechanisms. However, the symptom of daytime sleepiness is rarely observed in HF patients with OSA; thus, the diagnosis of OSA is easy to ignore. As the first-line therapy for OSA, continuous positive airway pressure(CPAP)can significantly improve sleep quality and attenuate daytime drowsiness. However, updated HF guidelines do not clarify the influence of CPAP on cardiac structure, function, and prognosis in HF patients with OSA. Therefore, this review summarizes the underlying pathogenesis of OSA in HF progression and reports on the related research status of the effect of CPAP on cardiac structure, function, and prognosis in patients with OSA and HF to heighten clinicians' attention to HF plus OSA and proposes the early screening and timely and effective diagnosis and treatment of patients with OSA and HF.
