    Study on single-nucleotide polymorphisms of microRNA and susceptibility to allergic rhinitis in Han population of southern China.

    Objective To explore the new molecular markers of susceptibility to allergic rhinitis by analyzing the relationship between single nucleotide polymorphisms(SNP)of microRNA(miRNA)and susceptibility to allergic rhinitis. Methods A total of 216 patients with allergic rhinitis were collected as the allergic rhinitis group, and 84 healthy people at the same term were collected as the control group. The peripheral venous blood was drawn in both groups, and genome DNA was extracted. The SNP of pre-miR-146a was detected using pyrosequencing technique to analyze its gene polymorphic site rs2910164 genotype and allele frequency. Results Both CC genotype frequency and allele C frequency of pre-miR-146a(rs2910164)in the allergic rhinitis group were dramatically higher than in the control group(P=0.031). Through analysis on the relative risk of genotype frequency, it was found that the risk of patients with CC genotype who suffered from allergic rhinitis increased(OR=1.799, 95% CI: 1.042-3.107), and the risk of those with allele C who suffered from allergic rhinitis also increased when compared with allele G (OR=1.348, 95%CI: 1.036-1.754). Conclusion The SNP of pre-miR-146a(rs2910164)may be associated with susceptibility to allergic rhinitis in Chinese Han population, and its mutation sites may be the molecular markers of susceptibility of allergic rhinitis.
    Differentially expressed proteins between nasal polyps and normal nasal mucosa by the iTRAQ technology.

    Objective To investigate the pathogenic mechanisms of nasal polyp(NP)by analyzing the protein expression profiles of nasal polyps. Methods 4 samples of NP tissues and 4 samples of normal nasal mucosa were retrospectively collected. The protein expression profile of each sample was detected by isobaric tags for relative and absolute quantitation(iTRAQ)technology. Results Out of 311 differentially expressed proteins detected, 88 cases were up-regulated, including Charcot-Leyden crystal protein(CLC)and PLEKHA7. Another 223 proteins were down-regulated, including S100A1 mainly. There were 18 cases and 65 cases of protein, with up-regulated expression more than two times and down-regulated more than 50%, respectively. Conclusion The protein expression profiles vary between NP and normal nasal mucosa up-regulation of CLC may involve in inflammation and immune response of nasal mucosa, and PLEKHA7 may be associated with mucosa reconstruction.
    Catgut implantation at acupoints regulate immunity of sd rat with allergic rhinitis.

    Objective To explore the effect of catgut implantation at acupoints on allergic rhinitis(AR)animals and to discuss the mechanism based on pathological morphology and immunology. Methods A total of 70 healthy SPF SD rats were randomly divided into the blank group, the AR model group, the AR western medicine group, the AR false catgut implantation at acupoint group and the AR catgut implantation at acupoint group. AR animal models were established according to the method of improved ovalbumin injection and nasal mucosa stimulation. Every group received different therapies except the blank group. After the last treatment day, ovalbumin nasal mucosa stimulation was carried out, then the symptoms were observed and recorded. SD rats were killed and septum mucosa was collected to detect expressions of tissue TGF-β1 and IL-17 by using immunohistochemical method. Results Catgut implantation at acupoints can obviously alleviate the symptoms of sneezing, runny nose and scratching of AR rats. Compared with model group(Ⅱ), catgut implantation at acupoints can reduce the TGF-β1 and IL-17 content in the AR rat nasal mucosa. False catgut implantation at acupoints can reduce the TGF-β1 content. Conclusion Catgut implantation at acupoints can reduce the TGF-β 1 and IL-17 content to regulate immune system in AR rat nasal mucosa, then alleviate AR rats symptoms.
    Study on measurement of nasal nitric oxide in children with allergic rhinitis.

    Objective To investigate the measurement of nasal nitric oxide in children with allergic rhinitis. Methods A total of 50 children(100 cases of nasal cavity)without nasal symptoms were selected as the control group between September 2013 and August 2014. Another 58 children(116 cases of nasal cavity)with allergic rhinitis between September 2013 and August 2014 were recruited as the allergy group. The nasal nitric oxide concentration of children from the two groups was measured by ‘passive nasal respiration’ technology. Results The concentration of nasal nitric oxide in the control group was(266±84)ppb and was(426±151)ppb in the allergy group(P<0.01). There was no significant difference in concentration of nasal nitric oxide between different genders(P>0.05). Conclusion The concentration of nasal nitric oxide may be affected by age and cooperation of children. The passive nasal respiration technology is a user-friendly and noninvasive measurement for initial diagnosis of allergic rhinitis.

    Objective To investigate the clinical efficacy of sublingual immunotherapy(SLIT)of standardized dermatophagoides farinae drops in patients with allergic rhinitis. Methods Totally 62 patients with moderate to severe persistent allergic rhinitis caused by dust mite received SLIT for two years. Dust mites and(or)house dust mite allergen skin prick index of all patients was ≥3+. Visual analog scale(visual analogue scale, VAS)and the maximum forced expiratory flow-volume curve(maximal expiratory flow-volume curve, MEFV)score were respectively applied to determine the nasal symptoms and small airway function before and after treatment. Results SLIT significantly reduced the individual symptoms like sneezing, rhinorrhea, nasal obstruction and pruritus after two years treatment when compared with the baseline, and the VAS scores of each symptom decreased significantly(P<0.01). The results of MEFV showed that MMEF, FEF25% and FEF50% were significantly improved after two years of SLIT treatment with significant differences(P<0.01). Conclusion SLIT of standardized dermatophagoides farinae drops has an favorite effect in patients with moderate to severe persistent allergic rhinitis.
    Improvement of the olfaction and quality of life of the chronic rhinosinusitis patients with olfactory disorders.

    Objective To explore the improvements of olfaction and quality of life(QOL)of the chronic rhinosinusitis(CRS)patients(with / without nasal polyps)with olfactory disorders. Methods 44 CRS patients with olfactory disorders were involved in this study and accepted CT scanning and Lund-Mackay CT score investigation before treatments. VAS score, SNOT-20 score and T&T test were taken before the operation, and 4 weeks and 12 weeks after the operation. Results In the T& T test, there were 59.5% and 73.0% of the patients whose olfaction were improved 4 and 12 weeks after the operation. The improvement rates were 77.3% and 86.4% in the VAS score. There were statistical difference between the pre-operation and post-operative 4 and 12 weeks(P<0.0001)in the T& T test and the VAS score, while the difference between the 4 weeks and 12 weeks after the operation were statistical insignificant(P=0.720,0.147). There was statistical difference between the pre-operation and post-operative 4 and 12 weeks(P<0.000 1)in the SNOT-20 test, while the difference between the 4 weeks and 12 weeks after the operation was statistically insignificant(P=0.055). Lund-Mackay CT score, T & T test, and VAS scores were positively correlated(r=0.569,P<0.000 1;r=0.782,P<0.000 1). Conclusion The olfaction and QOL of the patients significantly improve after endoscopic sinus surgery, and the difference between the 4 weeks and 12 weeks after the operation is statistically insignificant. The extent of the disease in the CRS and the degree of olfactory disorder are positively correlated.

    Objective To evaluate chronic rhino-sinusitis medication before treatment and quality of life(QOL)at different times after treatment and to investigate the drug therapy on QOL of patients. Methods Using sinusitis symptom visual analog scale(VAS)and sinusitis special scale-sinonasal outcome Test 20(SNOT-20), we collect 100 patients with chronic rhinitis-sinusitis as the experimental group receiving 12 weeks combination therapy drugs, once every four weeks, and another 100 healthy volunteers as the control group. Results By univariate analysis, the VAS score showed a significant difference(F=38.39, P<0.01)before and after treatment in the experimental group, and it showed a significant difference(t=7.93, P<0.01)between the 12-week after treatment group and the control group; After the drug treatment, VAS and CT score of 5 symptoms accounted decreases and each symptom rate also declined. By univariate analysis, the SNOT-20 score also showed a significant difference(F=27.93, P<0.01)before and after treatment in the experimental group, and it showed a significant difference(t=19.93, P<0.01)between the 12-week after treatment group and the control group;After the drug treatment, patents with each symptom accounted decrease and each symptom rate also declined. Conclusion Drug treatment can effectively improve the QOL chronic rhinitis sinusitis, whether needing long-term low-dose use remains further confirmation.
    Effect of NLRP3 inflammasome in the pathogenesis and relapse of eosinophilic nasal polyps.

    Objective To explore the possible effect and mechanism of NLRP3 inflammasome in the pathogenesis and relapse of eosinophilic nasal polyps(ENP). Methods According to HE staining, the nasal polyp tissues from 80 cases of ENP(ENP group)and 24 cases of recurrent essinophilic nasal polyps(RENP, RENP group)as well as middle turbinate mucosa from 30 cases of deviated nasal septum(control group)were selected. The number of eosinophil(EOS)infiltration in all patients was detected and observed by HE staining. The immunohistochemistry was used to detect the expression of NLRP3 inflammasome, IL-1β and IL-18 in three groups, and the correlation between NLRP3 inflammasome and the number of EOS infiltration was analyzed in ENP and RENP groups. Results The expression of IL-1β and IL-18 in both ENP and RENP groups was higher than in control group(P<0.05), but no significant difference was presented between ENP group and RENP group(P>0.05). The expression of NLRP3 inflammasome in RENP group was higher significantly than in ENP group and control group(P<0.01), and the number of EOS infiltration more than ENP group(P<0.05)and control group(P<0.01). The expression of NLRP3 inflammasome in both ENP and RENP groups positively correlated with the number of EOS infiltration. Conclusion NLRP3 inflammasome, IL-1β and IL-18 were all associated with the pathogenesis of ENP closely, and NLRP3 inflammasome may involve in the relapse of ENP.

    Objective To investigate the expression of neurokinin-1 receptor(NK-1R)in nasal mucosa tissue from patients with intranasal mucosal contact point headaches. Methods The expression of NK-1R was detected by immunohistochemical staining method in mucosal contact points and non-contact point from 18 patients respectively. The mRNA level of NK-1R and substance P(SP)was detected by real-time quantitative reverse transcription polymerase chain reaction(RT-PCR)at NK-1R positive cells at mucosal contact point and non-contact point respectively. Results NK-1R was mainly located in acinar epithelial cells, inflammatory cells and nerve fiber of contact point nasal mucosa. The mRNA of SP and NK-1R were up-regulated in mucosal contact point compared with those in mucosal non-contact point. Conclusion NK-1R, as a mediator of pain transmission, may play an important role in the occurrence of intranasal mucosal contact point headaches.
    Expression and significance of ERK5 protein in nasal polyps cells.

    Objective To investigate the expression and significance of extracellular regulated protein kinases 5(ERK5)protein in nasal polyps cells. Methods The clinical data of patients with chronic rhinosinusitis in our hospital from January 2013 to December 2014 was analyzed, in which 37 patients with nasal polyps were treated as the observation group(CRSwNP group)and 35 patients without nasal polyps were treated as the control group(CRSsNP group). Results Serum TNF-α(t=16.7, P<16.7), IL-5 (t=13.81, P<0.001)and IL-8(t=8.642, P<0.001)levels of the patients in the CRSwNP group were significantly higher than those in the control group, the difference had statistical significance(P>0.05). The results of western boltting test found that ERK5 protein expression level in the CRSwNP group was significantly higher than in the control group(gamma ratio 1.21±0.16 vs 0.36±0.04, P<0.01, n=37). By using Pearson correlation test ERK5 protein expression of the CRSwNP group had significant positive correlation with TNF-α(r=0.521, P=0.001)and IL-8(r=0.479, P=0.479)level. Conclusion ERK5 protein in patients with nasal polyps show high expression level and positive correlation with TNF-α and IL-8 level.

    Objective To explore the CT imaging features of the origin of nasal inverted papilloma. Methods All 47 cases of sinonasal inverted papilloma were confirmed by nasal endoscopic surgery and pathology. Their CT imaging data were collected and analyzed retrospectively.Bone changes of the tumor were observed and the relationship between the bone changes in CT images and the tumor origin were studied. Results Among 47 patients, 39 had imaging performance of bone density increase, including 37 of bone hyperplasia parts in accordance with the orgin of tumor and the consistency was 78.4%, suggesting that the orgin of the tumor could be speculated according to the bone hyperplasia parts. Conclusion Bone hyperplasia on CT imaging has a high consistency with the origin of nasal inverted papilloma,which helps to predict origin of tumor and to guide a reasonable scheme before surgery.
    Application of modified naso-pharyngeal tube in postoperative packing of FESS.

    Objective To study the improvement of postoperative discomfort by application of modified naso-pharyngeal tube when nasal packing after functional endoscopic sinus surgery(FESS). Methods A total of 162 patients undergone bilateral FESS in department of otolaryngology from January 2013 to March 2015 were enrolled in this study. 83 patients of them had been given nasal packing with modified naso-pharyngeal tube after operation(the experimental group), while 79 of them were not given naso-pahryngeal tube(the control group). All were recorded clinical indexes including operation time, and were evaluated and recorded by Visual Analogue Scale(VAS)for symptoms such as post-packing headache, nasal distending pain, sleep condition and pharyngeal discomfort. Results Post-operative 24 h VAS scores of the two groups showed the experimental group had better outcome in headache, nasal distending pain and sleep condition than the control group(P<0.05). While for pharyngeal discomfort, there were no statistically significant differences between the two groups. Conclusion Post-operative application of modified nasopharyngeal tube of FESS can effectively improve clinical symptoms such as postoperative pain.
    Effect of short-course budesonide suspension via nasal atomization on the operation bleeding of chronic nasosinusitis accompanied by nasal polyps.

    Objective To evaluate the effect of short-course budesonide suspension via nasal atomization on the operation bleeding of chronic nasosinusitis accompanied by nasal polyps. Methods A total of 96 patients with chronic nasosinusitis accompanied by nasal polyps were randomly divided into group A(n=33), group B(n=31)and group C(n=32). At 7 days before the operation, group A was inhaled budesonide suspension via nasal atomization and took roxithromycin; Group B was given budesonide nasal spray and took roxithromycin; Group C was given normal saline to wash the nasal cavity and took roxithromycin. The operations were performed in 3 groups according to the unified regimen. The amount of intraoperative bleeding in 3 groups and change of serum cortisol concentrations in groups A and B were compared. Results Average amount of intraoperative bleeding in 3 groups was(51.70±18.62)mL,(73.16±19.23)mL and(90.16±15.97)mL, and there was statistical significance among 3 groups(F=37.34, P<0.01). The concentration of serum cortisol in group A decreased after administration(P<0.01), but it was still within the normal range; There was no change in group B regarding the concentration of serum cortisol before and after administration(P>0.05). Conclusion Short-course application of budesonide suspension via nasal atomization can reduce endoscopic intraoperative bleeding of patients with chronic nasosinusitis accompanied by nasal polyp and has no obvious impact on the function of adrenal cortex.

    Objective To exlpore the prevention and therapy methods of sphenoid sinusitis after transsphenoidal pituitary adenomas resection. Methods Totally 129 patients underwent transsphenoidal pituitary adenomas resection. The postoperative sphenoid sinusitis was observed and analyzed. Results A total of 6 cases(4.6%)complicated with sphenoid sinusitis in all 129 cases. All of them were cured after the corresponding specialized subject treatment under nasal endoscope, like regular surgical cavity cleanning, nasal steroid spray, and nasal irrigation. Conclusion The complication of sphenoid sinusitis after transsphenoidal pituitary adenomas resection should be timely disposal under nasal endoscopy so as to effectively improve the patients clinical symptoms.
    Effect of endoscopic sinus surgery on airway resistance and olfactory function in patients with sinusitis and nasal polyps.

    Objective To study the effect of endoscopic sinus surgery on airway resistance and olfactory function in patients with sinusitis and nasal polyps. Methods Ninety patients with sinusitis and nasal polyps in our hospital from April 2012 to March 2015 were randomly divided into the observation group and the control group. The observation group received endoscopic sinus surgery, while the control group received conventional surgery. Then airway resistance, olfactory function, nasal cavity volume(NCV), nasal minimum cross-sectional area(NMCA)were compared between the two groups. Results 1, 4, 8 and 12 weeks after the surgery, the nasal airway resistance were significantly lower in the observation group than in the control group(P<0.05)and the olfactory function scores were significantly lower in the observation group than in the control group(P<0.05); 8 weeks and 12 weeks after the surgery, NCV and NMCA were significantly higher in the observation group than in the control group(P<0.05). Conclusion Nasal endoscopic sinus surgery can help to increase NCV and NMCA, reduce airway resistance and improve the sense of smell functions.

    Objective To investigate the relationship between sinonasal anatomic variation and chronic sinusitis in endoscopic sinus surgery. Methods A total of 206 patients with chronic sinusitis were selected. According to the presence or absence of chronic sinusitis, the patients were divided into observation group(n=85)and control group(n=121). The phenomenon of sinonasal anatomic variation including deviation of nasal septum, vesicular middle turbinate, inferior turbinate hypertrophy, agger nasi hypertrophy, reversed deviation of middle turbinate and uncinate process pneumatization was recorded in two groups. Results The incidences of deviation of nasal septum, inferior turbinate hypertrophy, agger nasi hypertrophy and vesicular middle turbinate in observation group were all significantly higher than those in control group(P<0.05), whereas there was no statistical significance between two groups by comparison to the incidences of reversed deviation of middle turbinate and uncinate process pneumatization(P>0.05). The upper deviation of nasal septum was very common in both groups, but the incidence of upper deviation of nasal septum in observation group was dramatically higher than in control group(P<0.05). Conclusion Deviation of nasal septum, vesicular middle turbinate, agger nasi hypertrophy and inferior turbinate hypertrophy all belong to the important structural variation in the development of chronic sinusitis, and are closely associated with the occurrence of chronic sinusitis.

    Objective To discuss the diagnosis and treatment method of laryngopharyngeal reflux disease(LPRD), and to observe the detection rate of helicobacter pylori from the patients with LPRD. Methods A Total of 156 LPRD patients with reflux symptom index(RSI)over 13 and/or reflux symptom index(RFS)>7 were supplied with 13C-Urea Breath Test(13C-UBT). The Hp positive patients(n=96, group I)were administrated with quadruple therapy(Omeprazole+Bismuth Potassium Citrate+Amoxicillin+Clindamycin)and Ganju Bingmei tablets for 2 weeks, followed up with a 4 weeks of withdrawal period. The primary ourcome observed was of Hp negative transformation rate and curative effect evaluation before and after the treatment; Hp negative patients were divided into group II and group III, with 30 cases in each group. The group II was given Ganju Bingmei tablets for 4 weeks and the group III was administrated with Omeprazole+Bismuth Potassium Citrate and Ganju Bingmei Tablets for 4 weeks. The main outcome was LPRD and HP negative-converting rate. Results The Hp negative-converting rate was 78.12%(75/96)in group I. The LPRD negative-converting rate in group I, group II and group III was 67.71%(65/96), 53.33%(16/30)and 96.67%(29/30), respectively.The score of RSI and RFS were decreased in all three groups after treatment, and dropped more significantly in group III(Statistic:6.73,P<0.001). Conclusion Laryngopharyngeal reflux, as an important cause of sphagitis, can be improved by using antiacid and gastrointestinal prokinetic agents.

    Objective To compare the clinical efficacy of cold dissection, monopolar diathermy dissection and radiofrequency ablation in tonsillectomy. Methods Patients undergone tonsillectomy were divided into 3 groups: cold dissection + suture group(group A, n=65), unipolar electric coagulation resection group(group B, n=40), plasma radiofrequency resection group(group C, n=38). The operation time, intraoperative blood loss, pain score, and postoperative albuginea of all groups were recorded and compared. Results The average operation time and intraoperative blood loss of group A, B and C were(58.8, 27.0 and 23.7)min,(34.2, 16.1 and 12.5)mL, respectively. There was significant difference in postoperative pain score 3 hour and 1 day after surgery. Conclusion Compared with cold dissection and monopolar diathermy dissection, radiofrequency ablation is more expansive, but it needs shorter operation time and causes less intraoperative bleeding and pain.

    Objective To investigate the clinical applications of bone-anchored hearing aid(BAHA)softband implanted in patients with bilateral congenital microtia. Methods BAHA softband was implanted in 56 patients with bilateral congenital microtia to improve hearing. The pure tone hearing sound field audiometry before and after the implantation were compared. Results After BAHA softband was implanted, the auditory functions of all patients were improved significantly. The pure tone hearing sound field audiometry was increased by an average of 32.9 dB. Conclusion BAHA softband implant can effectively improve audiometry without complications. It is a satisfactory solution for bilateral congenital microtia.

    Objective To observe the effect of deproteinized calf blood extract eye gel on corneal injury reparation after nasopharyngeal carcinoma radiotherapy. Methods A retrospective study was made for 97 patients who had received radiation therapy for nasopharyngeal carcinoma between January 2007 and December 2009 in Huaian First Peoples Hospital, Nanjing Medical University, with the radiation dose was more than 50 Gy. The right and left eyes were divided into the treatment and control groups,the deproteinized calf blood extract eye gel was given in the treatment group. The corneal endothelial cell densities, size, hexagonal cell ratio, and coefficient of variation were ddtermined at pre-treatment and post-treatment 1 day, 1 week, 1 month, 6 months, a year and 2 years in the two groups. All data were analyzed by statistical analysis software SPSS 12.0. Results At 1 month and 3 months after the treatment, the corneal endothelial cell density of the treatment group was significantly increased compared with the control group, the average corneal endothelial cell area was significantly reduced, the coefficient of variation of corneal endothelial cells was significantly decreased, the hexagonal cells of corneal endothelial cells were significantly increased, the difference was statistically significant(P<0.05). At post-treatment 3 months, changes of corneal endothelial cells in the control group began to decrease, while those in the treatment group was close to the pre-treatment state compared with the control group. The two groups had no differences in the rest time after radiotherapy. Conclusion Using deproteinized calf blood extract eye gel pre-treatment and post-treatment can help to reduce the damage to corneal endothelium and promote reparation of corneal endothelial cells.

    Objective To explore the clinical efficacy and safety of intravitreal injection of conbercept in the treatment of macular edema(ME)secondary to branch retinal vein occlusion(BRVO). Methods A total of 33 patients(38 eyes)with ME secondary to BRVO were selected as research objects, and injected Conbercept Ophthalmic Injection 0.05 mL through vitreous cavity. The optic examination was conducted at least once a month during 6-mongh follow-up. The central foveal thickness was measured to judge whether the injection was given again using optical coherence tomography(OCT). The best corrected visual acuity(BCVA)before and 1, 3 months after treatment as well as OCT results before and 1 month after treatment were compared, and the complications within 1 month after treatment were observed. Results The logarithm of minimum visual resolution(VA)was(0.59±0.21), and improved to(0.35±0.12)after the first injection(P<0.01). There was statistical significance regarding the BCVA before and 1, 3 months after treatment(P<0.05), and the BCVA 3 months after treatment was dramatically higher than that before treatment(P<0.05). The difference was statistically significant regarding the central foveal thickness of retina before and 1 month after treatment(P<0.05). Within 1 month after treatment, only 5 patients(15.15%)suffered from transient increase of intraocular pressure, no other eye-related events occurred. Conclusion For ME secondary to BRVO, intravitreal injection of conbercept based on OCT has definite short-term efficacy and better safety.

    Objective To evaluate the risk factors of diabetic retinopathy(DR)progression by observing the variation of visual acuity and fundus in type 2 diabetic patients after phacoemulsification and intraocular lens implantation. Methods A total of 50 patients with type 2 diabetes mellitus accompanied by non-proloferative diabetic retinopathy(NPDR)who needed to undergo phacoemulsification and intraocular lens implantation for the single eye were collected as research objects. Operative eyes and non-operatives were respectively as surgery and non-surgery groups. On 1, 7 days and 1, 3 months after surgery, both the best corrected visual acuity(BCVA)and optical coherence tomography(OCT)in macular region were conducted, and fundus fluorescein angiography(FFA)was also performed 3 months after surgery. The DR progression-related risk factors were analyzed. Results The visual acuity in surgery group improved obviously 1 d-3 months after surgery(P<0.01), but BCVA 3 months after surgery had the decreasing trend. The incidence of postoperative macular edema and DR progression rate in surgery group were higher than in non-surgery group, with statistical significance(P<0.05 or P<0.01). Logistic regression analysis research results displayed that glycosylated hemoglobin(OR=21.423), course of diabetes mellitus(OR=9.061)and NPDR degree(OR=2.486)were the risk factors of DR progression. Conclusion The BCVA of patients with NPDR in type 2 diabetes mellitus improves dramatically after phacoemulsification and intraocular lens implantation, but decreases slightly 3 months after surgery. Long course of diabetes mellitus, worse control of blood glucose and preoperative aggravation of DR are the risk factors of postoperative DR progression.
